Histamine and Head-Related Symptoms: What Happens When the Balance Is Disrupted?

Histamine does not act only in the digestive system. Through H1–H4 receptors, it also influences the nervous, vascular, and immune systems. In the central nervous system, histamine functions as a neurotransmitter and is involved, among other things, in regulating wakefulness and neural signaling.¹
Headaches, migraine-like symptoms, and dizziness have also been reported in people with suspected histamine intolerance. In a clinical study involving 133 patients, headaches and dizziness were among the frequently reported extraintestinal symptoms.²
Headaches and Migraine
Histamine affects both vascular tone and neural signaling. Activation of H1 receptors contributes to vasodilation and mechanisms associated with headache.³
The link between histamine and migraine is not merely theoretical. Experimentally administered histamine can trigger headaches and, in patients with migraine, may also provoke a migraine attack. The H1 receptor plays an important role in this process.³˒⁴
In some sensitive individuals, increased exposure to histamine may therefore be associated with headaches or a worsening of migraine-like symptoms. However, histamine is not the only mechanism involved in migraine, and it cannot be claimed that histamine intolerance is the cause of every migraine.⁴
Dizziness and a Feeling of Unsteadiness
The histaminergic system also plays a role in the peripheral and central vestibular systems, which are involved in maintaining balance and processing information about movement and body position. Histamine receptors have been identified both in structures of the inner ear and in central vestibular pathways.⁵
The importance of histamine signaling in the vestibular system is also supported by the fact that medications affecting H1 and H3 receptors are used in certain forms of vertigo.⁶
Through vasodilation, histamine can also affect blood pressure, which, with greater exposure, may contribute to feelings of weakness or feeling “faint.”³
This provides a biologically plausible explanation for a connection between histamine and dizziness. However, it does not mean that histamine intolerance is a general cause of dizziness. Dizziness can have many different causes.
Histamine and the Nervous System
Histamine is therefore not simply a substance associated with allergies and digestion. In the brain, it functions as a neurotransmitter and neuromodulator and plays an important role in regulating wakefulness, sleep, and the activity of other neural systems.¹
However, headaches, migraine-like symptoms, and dizziness are not specific to histamine intolerance. Clinical guidelines point out that symptoms attributed to ingested histamine are non-specific and that there is currently no single, generally accepted laboratory test that can definitively confirm the diagnosis.⁷
The symptoms described above therefore represent only some of the possible symptoms associated with histamine, and a single symptom alone is not sufficient to establish histamine intolerance.
